ACCESS Medicare Lifestyle Intervention Study

The CMS ACCESS (Advancing Chronic Care with Effective, Scalable Solutions) Model is a voluntary Medicare initiative designed to expand access to technology-supported care for individuals living with chronic health conditions. Through this program, eligible beneficiaries receive personalized support that may include remote monitoring, health coaching, medication management, and virtual care to help improve health outcomes while complementing the care provided by their primary healthcare team.

 

Who is Eligible: 

Patients must be enrolled in Original Medicare (Medicare Part A and Part B) and have one or more qualifying chronic conditions within an approved ACCESS clinical track, such as hypertension, diabetes, chronic kidney disease, obesity and/or overweight. Eligibility is confirmed by the participating ACCESS organization during enrollment, and participation is always voluntary.

What can you expect:

Participants will be randomly enrolled into an active group receiveing additional group education and a digital device for tracking health statistics or a control group continuing regular care with their medical team.  

Program Requirements:

Active group will be required to use digital device daily for health tracking and attend monthly group educational sessions led by our Registered Dietitians either in-person or virtual, receive monthly phone calls from Health Coaches to track adhearance to the lifestyle program.
